Registration/Enrollment

Patients must be registered, enrolled and eligible to receive services in our clinic. The first step is to complete an Application for Health Care Benefits (VA Form 10-10EZ). For more information, please visit our Eligibility page or read more about VA Health Benefits. Please remember to bring any private insurance information (if available) to the first appointment.

Primary Care

Each eligible patient is assigned a Primary Medical Care Provider. Together, with the clinic staff, they coordinate a patient’s care within the VA health system. Services offered through the McHenry Community Based Outpatient Clinic include:

•Diagnosis and management of your health care needs
•Health Promotion/Wellness Health Education
•Special Needs
•Senior Care
•Other Services
•Laboratory services.
•A pharmacist-managed Coumadin Clinic is available on site two days per week.

Women’s Health Services

We are committed to meeting the unique needs of Women Veterans by delivering the highest quality health care to each woman, while offering her the privacy, dignity and the sensitivity she deserves. Comprehensive primary care is offered to Women Veterans at the McHenry Clinic. Services include well women exams, preventive health care and gynecological services.

Social Work

The McHenry CBOC offers a community-based program:

•Readjustment Counseling
•Social Services Support
•Individual Therapy
•Family Therapy
•Family Education & Counseling
•Relapse Prevention
•Acute/Long-Term Inpatient Treatment

Mental Health Services

McHenry CBOC Mental Health providers have comprehensive treatment plans to best meet the needs of their patients. If medication is necessary for treatment, they will be prescribed and mailed

Monday, Tuesday & Wednesday: 8 a.m. to 3:30 p.m.
Friday: 8 a.m. to 11:30 a.m.

Referrals

A patient may be referred to the Captain James A. Lovell Federal Health Care Center (our parent facility) for specialty consultations, such as Audiology (hearing tests), Ophthalmology (eye exams), Cardiology (heart specialists), Pulmonary, Gastroenterology, etc.

The McHenry CBOC sees patients by appointment only. It is not an Urgent Care facility.
