  1. To send us any pending items, create a message to your provider and click on the add attachment button on the message. Upload your documents in the message and send them to us.

Scheduling Patient Instructions

Enroll in Relay Health:

1) Complete the PHA Survey (https://data.nmpchc.med.navy.mil/pha)

2) Have your DOD 10-didgit ID number and your Unit Identification Code (UIC) on hand.
Once completed, a customized education form is generated. You can save, print, or email this document for your personal use.

*** Upon completion of the EPHA please message Fisher Clinic via Relay Health to let our staff know your EPHA is complete. Fisher Clinic staff will message you with an appointment time for the PHA part 2.***

Separation/Retirement Physicals

  1. To schedule a Physical please call (847) 688-2469 ext. 89177 or 89230, or visit us at the clinic in the Physicals Department.
  2. Please fill out and print the following forms prior to the physical:

  1. On the day of your appointment, please report to Fisher Clinic Physicals Front Desk NO LATER THAN 0730 CST to visit our Ancillary Department before your appointment time, which is required. (You may also walk in any day prior to your appointment date to do the preliminary rounds)
  2. You will need to bring your medical record and be dressed in the Uniform of the Day (Working or Service Uniform is acceptable).
  3. If you plan on going to the reserves, please fast 12 hours prior. Water and black coffee are OK
  4. If you are filing for claims, you will need to schedule this appointment before you schedule your VA physical. Only fill out 2807-1 and Mr. Harold will be the claims point of contact at (224) 610-4138

Full Physicals and Pre Commissioning Physicals

  2. Please fill out and print the following forms prior to the physical:

  2. Please bring your MedPros. You will need the PDF file and not the DD form 2766. 
  3. On the day of your appointment, please report to Fisher Clinic’s Physicals Front Desk NO LATER THAN 0730 CST to visit our Ancillary Department before your appointment time, which is required. (You may also walk in any day prior to your appointment date to do the preliminary rounds) 
  4. You will need to bring your medical record and be dressed in the Uniform of the Day (Working or Service Uniform is accetable).
  5. Fast 8 hours prior (water and black coffee are OK)

Coast Guard PHA

1) To schedule a Physical please call (847) 688-2469 ext. 89177 or 89230, or visit us at the clinic in the Physicals Department. 
2) Please report to https://nmcpeh-hpwebsvr.med.navy.mil/HRA/pages/default.asp to complete your health risk assessment and print it out to bring with you to your appointment.
3) On the day of your appointment, please report to Fisher Clinic’s Physicals Front Desk NO LATER THAN 0730 CST to visit our Ancillary Department before your appointment time, which is required. (You may also walk in any day prior to your appointment date to do the preliminary rounds)
4) You will need to bring your medical record and be dressed in the Uniform of the Day
5) Fast 8 hours prior (water and black coffee are OK)

SARP Physical

  1. To schedule a physical please call (847) 688-2469 ext. 89177 or 89230, or visit us at the clinic in the Physicals Department
  2. Please come in the day of your appointment an hour before your appointment for needed lab work.

Operational Orders/Special Program Screenings

  1. Before an operational or overseas appointment can be made the service member must:
    1. Have Orders / Letter of Intent (LOI) to a specific duty station
    2. Have a completed PHA within 1 year
    3. Be fit for full duty 
    4. Have no pending medical or dental appointments
      - Bring a copy of LOI or Orders to Suitability Screening in 237 to start the Over Seas Screening Process.
  2. Optional (4-9): Please fill out each form online and then print it off.

    3. Fill out all paperwork: (1300/2 FRONT)(1300/1 FRONT)(1300/1 PART II)(2807 PG.1,2,3)
    4. Gather all immunization documentation
    5. Have titers drawn if not already drawn
    6. Bring in titers documentation
    7. Receive required immunizations based off of titers results
    8. Required immunizations include:
      1. MMR
      2. HEP A
      3. HEP B
      4. Varicella
      5. Typhoid (Required for Japan/Guam)
      6. Japanese Encephalitis Vaccine (JEV) Required for
    9. Dental:
      1. Be examined here at Fisher Branch Dental Clinic (Tel: 847-688-3331)
      2. Please bring all civilian dental records to Fisher Dental Appointment
    10. Medical:
      1. Be seen by provider
      2. Children must be seen by a pediatrician prior to and receive a copy of appointment along with most recent wellness check
      3. Females: Have up to date PAP smear and Women Wellness Exam (WWE). Please have documentation of both
      4. Children:
        1. Have school they are enrolled in fill out DD Form 2792-1

 Re-Enlistment

1) Before a Re-enlistment appointment can be made The service member must verify all documentation by Suitability Screening
1a) Have Orders to a specific duty station/LOI
1b) Have a completed PHA within 1 year
1c) Be fit for full duty
1d) Have no pending medical or dental appointments
2) Optional: Please fill out each form online and then print it off.

3a) For the 2807-1, please fill out each box and answer all questions with a yes or no.

Medical Assignment Screening (MAS)

1) Before a Medical Assignment Screening appointment can be made, the service member must verify all documentation by Suitability Screening:
1a) Have LIMDU Documentation stating off LIMDU
1b) Have a completed PHA within 1 year
1c) Be fit for full duty
1d) Have no pending medical or dental appointments
2) Please fill out each form online and then print it off.

4) Please also bring all Limited Duty documentations you have.

Individual Augmentation

  1. Before an Individual Augmentee Screening appointment can be made,the service member must verify all documentation by Suitability screening:
    1. Complete all paper work and clinical rounds here in Fisher Clinic
    2. Have a completed PHA within 6 months of deployment date.
    3. Have No pending medical or dental appointments
    4. Be fit for full duty
    5. Be off of limited duty
  2. Due to the in depth screening process, service members must physical come into Fisher Clinic, Suitability Screening Department, to book the appointment.
